PROGRAM:                    MCC Resettlement Assistance Program (MCC RAP)

POSITION:                     Health Navigator

REPORTS TO:              RAP Manager

HOURS:                           34 hours/week

SALARY:                          $32.29 per/hour

 

POSITION SUMMARY:          

The Health Navigator will play a vital role in facilitating access to healthcare services for Government-Assisted Refugees (GARs), ensuring they receive timely medical support upon arrival. This role involves close collaboration with internal teams, healthcare providers, and the Windsor-Essex County Health Unit (WECHU) to address immediate healthcare needs and improve long-term health outcomes. Additionally, the Health Navigator will develop and deliver culturally and linguistically appropriate health education sessions to empower newcomers with essential healthcare knowledge.

QUALIFICATIONS AND EXPERIENCE:

Education & Professional Training:

·       Bachelor’s degree in Public Health, Social Work, Health Sciences, Nursing, or a related field from a recognized institution.

·       Additional training or certifications in health promotion, community outreach, case management, or trauma-informed care are considered assets.

Work Experience:

·     Minimum of two years’ experience in community work, education, healthcare, or a related field, preferably in a non-profit or settlement services environment.
·       Experience in navigating healthcare systems, advocating for clients, and connecting individuals with medical, mental health, and social services.
·       Experience working with refugees, newcomers, and marginalized populations, understanding their unique challenges and healthcare needs.
·       Familiarity with crisis intervention and working with individuals facing medical, psychological, or settlement-related emergencies.

Healthcare & System Knowledge:

·       Strong knowledge of the Canadian healthcare system, including primary care, mental health services, and public health programs.
·       Understanding of healthcare barriers faced by newcomers, such as lack of insurance, language barriers, transportation challenges, and cultural differences.
·       Awareness of health determinants affecting refugee and newcomer populations, including trauma, mental health concerns, and chronic illnesses.
·       Experience in coordinating medical appointments, referrals, and follow-up care.

Client Service & Communication Skills:

·       Strong communication skills, both verbal and written, with the ability to explain complex healthcare information in an accessible and culturally sensitive manner.
·       Experience in group facilitation and delivering health education workshops to diverse audiences.
·       Demonstrated ability to build trusting relationships with clients while maintaining professional boundaries.
·       Experience working with interpreters and multilingual service delivery to ensure effective communication with clients.
·       High level of cultural competency and sensitivity in working with individuals from diverse backgrounds.

Problem-Solving & Organizational Skills:

·       Strong critical thinking and decision-making skills, with the ability to assess situations and determine the best course of action.
·       Ability to manage multiple cases and prioritize tasks efficiently in a fast-paced environment.
·       Proven ability to work both independently and collaboratively as part of a multidisciplinary team.
·       Excellent time management and attention to detail, ensuring accurate record-keeping and case documentation.

Technical & Administrative Skills:

·       Proficiency in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint) and experience using case management software or electronic medical records.
·       Experience with data collection and reporting, ensuring compliance with program requirements and funding guidelines.

Additional Requirements:

·       Valid Ontario Driver’s License and access to reliable transportation for client appointments and outreach activities.
·       Ability and willingness to work flexible hours, including evenings and weekends as needed.
·       Valid vulnerable sector police clearance, renewed every three years.

AREAS OF RESPONSIBILITY:

GENERAL

Client Support & Healthcare Navigation:

  • Assist clients in understanding and accessing the Canadian healthcare system, including mental health services.
  • Conduct needs assessments to determine healthcare priorities for newly arrived clients.
  • Schedule and coordinate medical appointments, tests, and follow-ups, ensuring language and transportation support as needed.
  • Refer clients requiring medical surveillance to WECHU and other healthcare providers.
  • Provide life skills training and health education to promote self-sufficiency in healthcare management.

 Collaboration & Community Engagement:

  • Develop and maintain relationships with healthcare providers, social service organizations, and community partners.
  • Work closely with internal programs to address client health needs holistically.
  • Participate in inter-program care conferences to ensure a coordinated approach to client support.

 Program Administration & Reporting:

  • Maintain accurate client records and program statistics.
  • Monitor client progress and evaluate service effectiveness.
  • Submit reports to supervisors as required and participate in team and community meetings.
  • Stay informed on healthcare policies and best practices to enhance service delivery.
